SUPPORTIVE OUTPATIENT PROGRAM (SOP)

THE PROCESS

  1. BOOK (Choose date and time). Date and time is not a commitment to a schedule. ( SORRY – we are working to remove this process)
  2. Make Credit Card payment. or schedule an appointment to make cash payment.
  3. Receive first email (Booking Confirmed – Your receipt).
  4.  INTAKE – Download intake form below and complete all forms/documents.
  5. Call office to go over paperwork. E-mail your complete forms, picture ID and paperwork to admin@trueoutreachinc.org
  6. Orientation – Enrollees will receive an orientation package in the portal. The package will go over basic rules (Facilitator will go over all rules on the enrollee  first day of attendance), class requirements, and give zoom log in links to all sessions. 
  7. Call office  to complete orientation and to be released to start attending session. At this point, an enrollment agreement will be completed and sent to the enrollee C. S. O. and to each enrollee.
  8. START attending the sessions immediately.

FAQ ANSWERS

  • Our Supported Outpatient Program (SOP) is taught VIRTUALLY – Online by ZOOM. No in-person sessions offered!
  • 18 sessions – 2 sessions per week for 9 weeks – each session is  4 hours .
  • Sessions are offered 8 times per week ( 9AM – 6PM Monday through Friday. Enrollees will be given access (zoom log in link) to each session.
  • Enrollee may only attend 1 session per day and no more than 2 sessions per week  maximum. 
  • Session attendance is flexible. for example, an enrollee may attend  weekly classes in the day and evening. Enrollee are to indicate preferred schedule while filling enrollment form.
  • Maximum time allowed to complete this course is 12 weeks from booking date (day of initial payment). If all 18 sessions are not completed within the max time allowed (12 weeks), the enrollee will be TERMINATED from the course.
  • SOP is an open group meeting. Enrollees will start immediately after completing all enrollment requirements. There is no set start date. 
  • Total cost for SOP course is $850.
  • $250 Down and $150 per month for 12 weeks. 
  • 4 payments of $150 each.
  • Monthly progress reports sent to C.S.O.
  • State approved completion certificates given to all graduates.
  • Taught in English only!
